New Clients and Patients
Welcome to Redbud Animal Dermatology!
To help us prepare for your pet’s consultation, please complete the new client & patient forms below.
We request that forms are completed no later than noon the Thursday prior to the week of your new client appointment. This allows ample time for our medical team to become familiar with your pet's history and prepare for your appointment. If the documents are not received by this deadline, then your appointment time may be cancelled and require rescheduling.
Additionally, please ensure your referring veterinarian sends us your pet's medical records, including previous diagnostics and treatments, prior to your appointment. These records are essential for our team to thoroughly review your pet's medical history and provide the most informed, efficient consultation possible.
New client forms can be completed from any computer, smart phone or tablet.

Existing Clients and Patients

To help us prepare for your pet’s recheck consultation, please complete the recheck client & patient forms below.
Please include any updates or changes to your pet's condition, medications, treatments, or medical history since their last visit. If your pet has received additional veterinary care or diagnostic testing elsewhere, please ensure those records are sent to our office prior to your appointment.
The form must be returned no later than 48 hours prior to your scheduled recheck appointment. This allows our team adequate time to review your pet's progress and prepare for your consultation. If the documents are not received by this deadline, then your appointment time may be cancelled and require rescheduling.
